  • Read More Nitrous Oxide Sedation
    Nitrous oxide (laughing gas) is an extremely helpful adjunct for dental treatment of the fearful patient. It was first discovered by Joseph Priestley in 1772, however, it was not until 1844 when Horace Wells used it as a medical anesthetic. Today, nitrous oxide is used in conjunction with local anesthetic when performing dental treatment on apprehensive patients. Nitrous oxide on its own cannot reliably anesthetize a patient profoundly enough for a patient not to feel a thing. Therefore, it must be used with novocaine to numb the area. Nitrous oxide is administered with pure oxygen and is breathed through a nasal hood. The machine that delivers the gas is a fail-safe machine which will turn off if the oxygen level falls below 35%. Nitrous oxide is not meant to put one to sleep, but merely relax. When given before the anesthetic injection, it can make that part of the procedure much more comfortable. It will put a patient in a state of mild euphoria and will alter the perception of sights, sounds, and time. It also has a mild amnesic effect leaving much less memory of the procedure. Another extremely nice feature of nitrous oxide is that the patient can be flushed with pure oxygen after the procedure and be totally lucid and able to drive home. Nitrous oxide can make a dental appointment a very pleasant experience. Read More

  • Read More IV Sedation in Dentistry
    It is not unusual to have some apprehension before a dental appointment. Most, if not all, of that normally goes away once the area to be treated has been numbed. However, some people have dental phobias, usually related to past, unpleasant experiences that make even a routine dental visit a stressful occasion. For most of these people, either nitrous oxide and/or oral sedation is adequate to quell the fears, but there are still a few who need deeper sedation. There are a number of wonderful medications that can be delivered intravenously. Since these are given IV, they can be titrated (adjusted) to the perfect level for each individual patient. We use Versed (midazolam) in our office. It is a very safe sedative that can fully relax a patient, but keep their protective reflexes and their ability to respond to commands intact. We monitor vital signs continuously to assure the highest level of safety. It is important that the patient does not have anything to eat or drink after midnight. They also must have a ride home after the procedure and should not drive or operate machinery for the next 24 hours. The addition of IV sedation to a dentist's armamentarium can make a dental procedure extremely pleasant for even the most fearful patient. Read More

  • Read More Root Canal Retreatment
    A root canal is not something that we normal humans would look forward to. How about the poor people who have to have a root canal retreated? It’s a common perception that once a root canal has been done, there will never have to be anything else to do for that tooth. While that is true with most root canals, there is an occasional one that is not successful. The most common reason is not being able to get a good seal on the root. Any leakage around the root canal filling material can allow bacteria to filter in. Since there are no blood vessels in the area (those were removed by the root canal process), there will be no way to get antibodies and white blood cells to the area. This way, the bacteria can proliferate causing an infection. The only way to correct this problem is to retreat the root canal. That is done by opening up the tooth, then softening and removing the gutta percha filling material. After that is done, it’s a good time to search for extra canals or unusual root anatomy. All of the canals are cleaned thoroughly and irrigated with sodium hypochlorite. At this point, a decision is made on whether to place medication in the tooth and allow it to heal for a week or two or to go ahead and fill it that day. Retreatments do not enjoy the same success rate as original root canals, but they work often enough that they are sure worth giving a try. Read More

  • Read More Periodontitis
    Periodontitis is a disease of the gums and causes a majority of the tooth loss in adults. The sad part is that over 90% of the cases are preventable. Periodontitis is caused by an interaction of bacteria which reside on the teeth and the body’s immune system. The bacteria adhere to the teeth by a sticky substance called dental plaque. The plaque protects the bacteria from antibodies in saliva, thus allowing the disease to progress. Simply by removing the dental plaque everyday would stop the disease dead in its tracks. Mild periodontitis is the first step past gingivitis, a reddening of the gums. It is marked by easily bleeding gums, especially when brushing the teeth. If good oral hygiene measures are initiated, many times the disease process can be reversed. Moderate periodontitis is accompanied by some bone loss. If caught at this time a procedure called scaling and root planing can be helpful in stopping the disease progression. From this point on, any bone damage is irreversible, so it is best to catch things as early as possible. Severe periodontitis is the point where the teeth will loosen and tooth loss can occur. These cases usually require surgery to give the sufferer access to all of the affected areas for the purposes of oral hygiene. Due to the irreversibility of the bone loss, it is always best to put the disease in check by daily home care and frequent professional checkups. Read More

  • Read More Tooth Trauma (Part I of IV)
    Whenever a tooth is injured in a fall, there are certain steps that one must take. First and foremost, a head injury must be ruled out. When one has fallen or received a blow to the mouth, it is not uncommon to also incur trauma to the head. Especially when a tooth has been chipped and there is blood in the mouth, the focus of attention can be on the oral cavity. If there are any symptoms such as dizziness or loss of consciousness, a physician should be consulted. Once head injury has been ruled out, then the focus can be placed on the mouth. If a front tooth was chipped, look for a small red dot toward the center of the tooth. This should be checked after any blood has been rinsed away. The red dot is the tooth's nerve. If it is exposed, there is a fairly high likelihood that it would need root canal treatment. A dentist should be consulted as soon as possible in that situation. The next thing to look at is to see if any teeth are loose or displaced. Again, if either of those conditions are present, a call to your dentist would be in order. Otherwise, it would be best to see your dentist at your earliest convenience so that he can x-ray the tooth to make sure that there are no fractures under the gums. Read More

    The big day has finally come, after all of the impression and try-in appointments, the actual dentures will be delivered. Upon first inserting the dentures into the mouth, they may feel tight and foreign. Sometimes, there are little spicules of plastic that come from voids from air bubbles in the model. These can easily be removed. The next thing to check is the bite. Even though the bite is verified during the try-in appointment, there are processing errors that creep into the plastic which can cause the bite to be off. Once the bite has been adjusted, then the cosmetics can be checked to make sure that they didn't change from the try-in. Then a final check is done by moving the lips and tongue to make certain that the denture does not become dislodged. Now it's time to take them home and try them out for real. It is important to not have unrealistic expectations. For new denture wearers, one must understand that they will not function the same as natural teeth. The chewing power is not near as good and they can become dislodged by certain movements of foods. For those who have worn a previous denture, the new denture will not feel the same as the old one. It's just like getting a new pair of shoes. Even though the old pair has holes in them and are falling off your feet, the new pair usually feels very different. But when one walks in the new shoes for a few days, they will feel totally natural. This is the same with dentures. Sometimes you have to force yourself to wear the new set because they feel so foreign, but after a few days, they should function naturally. Read More
