Dr. Gangwisch – Hello. I’m Dr. Richard Gangwisch. Today we’ll be placing four laminate veneers. Now in the previous video, we had prepared these teeth to be able to accept these veneers. We made a mold, sent that to the lab and the lab has made some absolutely beautiful teeth for her. So today, we’ll try them in and we’ll make sure that everything looks great and then we’ll cement them in permanently. Now, these laminates will be extremely durable and color stable and they will last for many years. And our patient will walk out with a dazzling Hollywood smile. Well Sherry the big day is here. So what do you think about your temporaries? Do you like the lack of spaces now?
Sherry – I do like the lack of spaces. I love them!
Dr. Gangwisch – Are you excited today?
Sherry – I’m very excited!
Dr. Gangwisch – Well, that’s great! Well let’s get started!
Sherry – Great!
Dr. Gangwisch – All right! So first of all, we’re going to get started and go ahead and take the temporaries off. They just kind of pop off. Actually look pretty good there. This is the actual porcelain laminates. They came back from the lab. So we have to get all the plaque and anything else off the tooth surface, so it is good and clean before we cement. Now we are going to try them in. Got to make sure everything looks good and fits good before we continue. We will try them in individually first to make sure that everything is fitting in independently of the others. Then, since of course we are going to end up with them all in place, we make sure they are all going to fit in at the same time. So we’ll try all those in. Make sure the alignment is nothing less than perfect; looking pretty good at this point in time. I think this patient will be happy. Alright Sherry! The big moment. Sherry – They look great!
Dr. Gangwisch – Like that?
Sherry – Yeah! Awesome!
Dr. Gangwisch – All right! Now we’re going to go ahead and actually place the laminates permanently on the teeth. Now this blue material here is actually an acid preparation. What that’s going to do is it’s actually going to take a microscopic layer of enamel off and just enough so it will cause some little tiny crevices in there. What we’ll do is we’ll put some liquid plastic over that and allow it to flow into place and then it will harden and that’s actually what holds that on. We’re not gluing these on to the teeth. We’re actually bonding them by mechanical retention. It’s not like using super glue or something like that on them. Dry them up good and make sure everything is spotlessly clean and well etched. Some people have some sensitive teeth and so I’m applying a desensitizer here. You never know who’s going to have the issues with it and it’s certainly nice to do this ahead of time, just in case. This doesn’t affect the bond at all. Now we have to get all of the moisture out of there to make sure that we’ll have a perfect bond. Now we are applying the liquid plastic to those little crevices and that’ll flow in. It’s such a small amount of enamel that’s missing with that. Really can’t see it except for under a microscope. So now we are actually cementing the actual laminates into place. It’s a resin cement that we are using and it’s kind of a little bit like the tooth colored filling material that we fill teeth with only not as viscous so it will allow for the laminates to seat properly. I like to seat those first two together so that way we make certain that we get perfect cosmetics. Got to get the esthetics right on. Going to press them into place so we can allow some of the excess cement to exude out. We have to check that fit, always checking that because once you apply the light to it that will be it. Now by the light, it’s a photo cured material that we are using, so it allows us all the time in the world to get it just right and then when we are ready to finally have it set then we zap it with the light and it sets. It sets rock hard. That’s why we are trying to get some of that excess off the brush there. Got to check that alignment again and make sure everything is perfect. Final check for the fit. Feel pretty comfortable about that. Time to put them in place permanently. That’s the light that’s curing everything. All right now we have to check the fit of the other two even though we have checked it once. Sometime you can get a little bit of movement now those two middle ones are in permanently we have to check again and make sure it fits good. So we do the same sequence here. We’ll go ahead and etch the enamel and place the liquid plastic onto them. Allow that to harden and bond the laminates in place. Cementation time now. Make sure those go into place well. Check the edges right at the margins. Make sure it’s nice and smooth. Get the excess; the more excess we get off, the less problem there is in cleaning because when that sets, it is rock hard. Things look good so we’ll zap away again. So clean up time again. So now we’ll go in between the teeth again and get some of that excess cement off. Floss there so we make sure not to give her an excuse not to floss. Go gently along the gum line to get the excess cement off. Can’t leave any of that up underneath the gums. Check the bite. Looking good. Now the big moment here.
